Holby City spoilers follow from tonight's episode.

It's the twist you probably did didn't see coming – Bernie is alive. And she's back in Holby City.

The closing moments of Tuesday night's episode (August 31) unveiled the biggest twist of the year so far, but in an even bigger shock, Bernie's evil son Cameron was knocked over before he had a chance to react to her resurrection.

But why is Bernie back? Will Cameron be okay? And what in the world is going to happen next? Here are all the questions we've been left with until next week:

1. Why is Bernie back?

Hold the front door, Bernie is back from the dead – alive, (reasonably well), and ready to turn Cameron's story on its head.

The closing moments of tonight's gobsmacking episode saw Bernie emerge from a car, over two years after she was claimed to have been killed in action.

Jemma Redgrave, who bowed out of the show in 2018, has reprised her role as the popular medic for a huge, new story. But where has she been? Why is she back? Who exactly was buried instead? And what will all this mean for Cameron if he lives to tell the tale? Speaking of which...

2. Will Cameron be okay?

And if Bernie's return wasn't shocking enough for Cameron, there's now a very good chance that she'll never get to explain herself to him anyway.

In an unfortunate twist of fate, a shell-shocked Cameron was mowed down by an ambulance seconds after seeing his mum rise from the dead, meaning their reunion could be extremely short-lived.

Will Cameron survive? And what will Bernie make of her son's killer antics regardless if he does? So. Many. Questions.

3. Is the hospital in danger?

Let's be honest, Cameron's return has been nothing short of a farce so far, and we don't believe for one second that he would have legitimately been able to fake his death as he did.

Ignoring the nonsensical nature of the plot, though, Cameron was on the loose for the entirety of tonight's episode and there's now a very real chance that the hospital is in danger as a result.

As part of his ultimate revenge plan, Cameron placed a bomb on the hospital grounds shortly before his accident, meaning his former friends and colleagues could now be very much on borrowed time.

Bernie being back in the vicinity could change everything for Cameron, but is it too little too late?

4. What next for Jac?

Away from the Cameron chaos, Jac's sad story gathered momentum tonight, as she got further unsettling news regarding her health.

Jac recently told Hanssen that she'd been diagnosed with a brain tumour prior to returning to Holby, but she's now discovered that it's more serious than she thought.

In turmoil, Jac finally opens up to Max about her prognosis and explains that she's likely to go blind. She also used the opportunity to admit the truth about the stent trial and her personal reasons for driving it.

Max urged Jac to face up to reality, and pull a deteriorating Carole from the trial, but what does the future hold for Jac now?

5. Is Evie in too deep?

Evie faced a new dilemma tonight when Aleysa, another of Jeni’s girls, was admitted with a suspicious cut on her arm.

When Fletch and Donna didn't buy the reasons behind Aeysa's injury, Jeni blackmailed Evie into telling more lies.

Evie then saw an even darker side to Jeni when the police were forced to get involved. But with Fletch's freedom at stake, is Evie simply in too deep now?
