Holby City spoilers follow.

Next week's slightly unconventional Holby City episode (July 14) will see Nicky forced to make some difficult decisions, Fletch and Essie face the stark realities of their illness and Guy is pushed to the brink.

Here are seven of the biggest spoilers...

Viewers will notice a slight change in house style throughout next week's episode, as it plays around with time in a bid to give Nicky, Fletch and Guy's big stories some proper momentum.

The start of the episode will show three game-changing scenes between the main players mentioned above, before going back in time to four weeks prior to show us the circumstances that led up to that point.

2. Nicky recovers in hospital

Nicky is recuperating in hospital when we catch up with her next week, having suffered a heart attack in last night's shocking scenes.

As we revealed above, next week's episode will span a four-week timescale which tracks Nicky's recovery – and the difficult decisions she has to make as part of it.

3. Tracey causes more problems

With Tracey keen to keep a close eye on Nicky following her heart attack, she unwittingly (but not entirely surprisingly) ends up becoming more of a hindrance than a help.

Having moved in with Nicky, Chloe and Cameron, Tracey's overbearing presence soon causes a rift between the trio.

After Chloe and Cameron make their feelings known, Nicky sides with her mum, but will it be at the cost of her friendships?

4. Fletch gets worrying news

Next week's episode is also a big one for Fletch, who has, up until this point, been confident that he was on the road to remission.

Unfortunately, Fletch will face a big setback when he gets some concerning news from his consultant and fears his cancer is back.

Fletch then faces a painful week's wait to get answers, but what will they bring?

5. Fletch and Essie befriend a patient

As they continue with their treatment, Fletch and Essie make friends with a young lad called Jasper, who has also been diagnosed with cancer.

While Fletch and Essie desperately try to remain upbeat about their cancer diagnosis, they are inspired by Jasper's care-free and positive attitude towards his own.

However, when faced with the stark realities of the illness, all three of them struggle to cope – leaving them to realise that Jasper's brave front is merely a coping mechanism. But can they support each other?

6. Nicky makes a heartbreaking choice

With Nicky keen to come back to work, she is left feeling frustrated when Jac repeatedly insists she simply isn't ready.

Keen to get Nicky to accept the reality of what's happened, Jac urges her to think about the most stressful aspect of life – and remove herself from it.

While Tracey is adamant that the cause of Nicky's issues all begin at the hospital, the junior doctor is forced to reassess what she really needs when her mum comes in between her work and home life.

As Nicky confronts her fears, she is urged by Jac to make a heartbreaking choice. But what will it be?

7. Guy struggles to regain control

Guy pledges to be the man he once was by doing a risky operation on a patient and thus proving his brilliance as a neurosurgeon.

Despite the recent tensions between them, Guy even manages to get his sceptical ex-partner Max to agree and it's all systems go as he later prepares for theatre.

However, when the surgery goes drastically wrong, Guy is left on the brink of despair and is tempted back to worrying old habits...

Holby City airs these scenes on Tuesday, July 14 at 8pm on BBC One.
