8 Holby City spoilers for next week

Holby City will air one of its most outlandish episodes of all time next week, as surgeon Ric Griffin is faced with some ghosts from his past when he goes under the knife.

As promised, the show will be delving into anomalous, unconventional and slightly quirky territory for Ric's special story, which will go beyond the realms of the four walls of the hospital.

While Ric is facing a life-or-death situation, it's make or break for Dom and Lofty when Helen returns. Here is your full collection of the biggest spoilers:

1. Donna is panic-stricken

As the day of Ric's operation arrives, his friend Donna is desperate to support him – but gets a shock when she realises just what his risky procedure entails.

The surgery has never actually been successfully tried on a human being and comes with multiple risks, including a stroke or death.

Although the hospital's new neurosurgeon Max is adamant she knows what she's doing, Donna shares Serena's concerns about whether this is really the right option for Ric.

With his mind made up, Donna says her goodbyes to Ric before he is taken down to surgery and then faces an anxious wait to see if he pulls through.

2. Ric makes Serena a promise

As he lies on the operating table, Ric shares an emotional exchange with a worried Serena – who wants him to make a very important promise.

Acutely aware of the risks of the high-stakes surgery, Serena begs Ric to come back to them afterwards and assures him that they will all be waiting for him when he does.

Although Ric does indeed promise Serena he will wake up, a mystifying turn of events later on mean that he may have to go back on his word...

3. Max performs dangerous brain surgery on Ric

Despite reservations from a number of her new colleges, Max ploughs ahead with the procedure on Ric regardless.

Unfortunately, things take a worrying turn amid the ultrasound, prompting Max to perform highly dangerous brain surgery on Ric – against his previous wishes.

With Ric's life now hanging in the balance, Max teams up with Serena as they desperately try to rectify the increasingly urgent situation. But will he pull through?

4. Diane is back...

As we've already revealed, Ric's story is going to tie in with the return of the late Diane Lloyd – whose comeback "goes beyond the realms of the hospital" and into very different territory for the show.

Patricia Potter has reprised her role as Diane for a one-off appearance, following the character's devastating death in 2007. And if you're already a bit confused, trust us, you haven't seen anything yet...

5. And so is Ric's son Leo

It's not just Diane who is making a spooky and slightly eccentric return from the dead, as Ric will also be confronted by his late son Leo in next week's episode as well.

Long-term viewers of the show will know that Ric's eldest child died of a drugs overdose in a harrowing story back in 2010, but Daon Broni has reprised his role as Leo for the unconventional scenes.

Leo has an important message for his dying dead, but what is it? And will Ric listen?

6. Ric faces his own mortality

As Ric's health rapidly declines in the 'real' universe, fans will see him facing some very big decisions in an alternate one.

Intertwining the reality of Ric's surgery with the dream-like sequence of his subconscious, this special episode will play into all of the medic's big life dilemmas – past, present and future.

As Ric confronts his mortality and his past burdens, both Diane and Leo force him to make an all-important choice. But will Ric choose life or death?

7. Helen returns to Holby

She's the iceberg that threatens to rock the Dofty ship, but a heavily pregnant Helen is back in Holby next week. And this time, her secret is more difficult to hide.

When Helen is brought up onto the ward following a car accident, she begs Dom to be discreet when it comes to her pregnancy – but it's not long before Essie is questioning their connection.

After Dom confides in Essie, she urges him not to keep Lofty in the dark – reminding him of the damage secrets have already done to their marriage.

With Dom torn between patient confidentiality and loyalty to his husband, the decision is taken out of his hands when Lofty spots Helen on the wards – along with her very visible baby bump.

8. Lofty and Dom hit the rocks (again)

Following his discovery about Helen and their baby, Lofty understandably has some very big decisions to make over his future.

After vowing to stand by Helen and raise their child together, Lofty is left in another predicament over where this leaves his relationship with Dom.

Hurt over being kept in the dark for so long, Lofty confronts Dom over his months of deception – but can he forgive him?

Holby City airs these scenes on Tuesday, October 22 at 8pm on BBC One.
