Emmerdale fans heartbroken over Cain and Ruby twist

Emmerdale spoilers follow.

Emmerdale viewers were left in shock at Cain Dingle's betrayal of Moira in new dramatic scenes.

In tonight's (November 20) episode, Natalie J Robb's character underwent surgery for her brain tumour, whilst her husband Cain struggled to cope with the thought of losing her.

In a devastating turn of events, Cain cheated on Moira with his brother Caleb's wife, Ruby Fox-Miligan.

The episode was divided into two parts, with the first half focusing on Moira's visions while she lied on the operating table. Under anaesthesia, she had a bittersweet reunion with her late daughter Holly Barton.

The tearjerking moment was cut short when Moira woke up after surgery to a nurse calling her name and telling her the operation had gone well.

Later on, she opened up on her dream about Holly to Cain, who was visibly upset and smelled of whisky.

The second half of the episode then played in reverse chronological order, revealing what Cain had been up to when Moira was having surgery.

The clock went back four hours to show a drunk Cain walking around the village after a few upsetting encounters, including one with Jimmy King, who insisted Belle was lying about being abused by Tom.

Earlier on, Cain hid in the garage while Moira was fighting for her life in the theatre. Ruby found him there and urged him to go see his wife, but their confrontation unexpectedly escalated in a moment of physical passion.

"No one finds out about this," Cain told Ruby as he put his clothes back on afterwards.

"You're damn right they don't," she replied hurrying out.

A guilt-ridden Cain headed to the hospital, where he was informed Moira had suffered a seizure during the operation, but pulled through and was awake.

Fans of the soap were less than impressed with the cheating twist and took to Twitter/X to voice their outrage.

"CAIN AND RUBY??? no please no #Emmerdale," tweeted one viewer. "Cue Television's across the UK flying out of windows! How could you Cain?!" another wrote.

"Oh, Cain. You’ve let Moira down. You’ve let yourself down. You’ve let me down. #Emmerdale," commented a third fan.

But what does Cain's infidelity mean for the fan-favourite couple?
