Holby City brought the continuation of Jac Naylor's mental health problems tonight (November 5) as Elliot Hope made a surprising return to Holby.

Last week, Jac (Rosie Marcel) got the news that ex-husband Jonny and wife Stacey were moving to Edinburgh, with Stacey making a threat that Jonny would want full custody over Jac and Jonny's 5-year old daughter Emma.

Stacey's bombshell was the start of Jac's mental health falling apart and Tuesday's episode picked up on last week's events as Jac looked visibly distressed.

The 1,000th episode of the BBC One drama marked a huge milestone for Jac as she was about to perform her 1,000th heart-transplant operation, while she also reunited with old friend Elliot Hope (Paul Bradley).

While Elliot didn't immediately reveal why he was back, he was shocked to see how much Jac had changed.

Elliot's suspicions proved right when he witnessed Jac's problems interfering with her judgement about a patient's operation.

Things went further when Jac took Emma down to the mortuary as a punishment for her playing around with a patient, but a scared Emma ran off and almost got hit by an ambulance. This was an instant wake-up call for Jac as she started considering whether she should give up Emma.

Later on, Elliot confronted Jac about Emma, and asked for an explanation: "[Emma] said you were in the basement. Please tell me you weren't going to the mortuary."

"She's 5 years old," he added. "What is going on with you, Jac?" But Jac turned her back and ran away, refusing to talk about what happened earlier.

The episode's end saw Jac telling Emma that she would be soon moving to Edinburgh with her dad and stepmum, which indicated that Jac decided to give up on Emma's custody.

Viewers watching at home were simply heartbroken over Jac's devastating decision.

Holby City continues next Tuesday (November 12) on BBC One.
